Member Notices

Medicare Part D Creditable and Non-Creditable Coverage Letters
If you have changed your Pharmacy Benefit since November 2020, you will also need to review your current Pharmacy Benefit to determine whether it is creditable or not before choosing which letter to send. You can obtain current copies of the Creditable and Non-Creditable letters at the link above. This notice must be provided to all eligible employees on or before October 15, 2022.

Women’s Health & Cancer Rights Act Notice
The Women’s Health & Cancer Rights Act Notice must be sent on an annual basis and must be distributed by the end of the year, December 31, 2022. The form has not changed, so you can use the notice that you used in prior years or obtain a copy of the notice at the link above.

CHIPRA Special Enrollment Notice
All Plans are required to send out a notice to all employees (not just those covered under the Plan) regarding their rights to enroll their children under your health plan if they lose coverage under Medicaid or a state-sponsored child health insurance program. The Notice for this requirement can be found at the link above. The form has been revised and the new version should be used. This notice must also be distributed by the end of the year, December 31, 2022.
